Does that fit an old Buick 215 flywheel (with thick outer ring), or would someone need to buy a D&D (flat, Chevy bolt pattern) flywheel to go with it?

I believe you’re correct on the flat flywheel. . For reference, the clutch mounting hole pattern has 3 sets of 2 holes at 3” spacing on a 11” bolt circle
